Green Historical Society Scholarship Release
This form must be completed by any student wishing to compete for the 2011-12 scholarship award. This form must be completed and returned to the Green Historical Society via email at [email protected] or via post at Green Historical Society, P.O. Box 533, Green, Ohio 44232. This form must be received by the Green Historical Society prior to the scholarship distribution.
